First, you'll need to install the PS4 Remote Play app on yours iPhone / Ipad. It is compatible with both devices. You'll also need to make sure your Apple devices and PS4 are on the same network Wi-Fi.
After installing the app, turn on your PlayStation 4 and make sure Remote Play is enabled. Enter the Settings menu: it's the icon that looks like a small suitcase.

Do not exit this menu yet, as you will need it in the next few steps. For now, however, switch to your iDevice and launch the PS4 Remote Play app.
As soon as it starts up, a simple screen appears with a big old Start button. Tap it, then log into your account Sony.
Once you enter the app it will immediately start looking for yours PlayStation.
Warning: your PS4 will not be found right away. Sometimes it will take multiple tries, so to make things easier, go back to your PS4 and click on the “Add Device” menu (also in Remote Play Connection Settings). A code will be displayed.

Go back to your iDevice, tap the "Register Manually" button and then enter the code shown on your PS4.

That's all there is to it. It should connect and you will be ready to play. You only need to do this once, after which you should be able to start remote play anywhere, even if you are away from home.
Once you've got everything set up, you might want to take a quick second to make sure you're getting the best experience.
From the main Remote Play screen (before connecting to the PS4), tap thegear icon in the upper right corner. The main thing you'll want to focus on here is the Video Quality option for remote play, which will determine the streaming graphics quality.

The resolution for remote playback is set to Standard (540p) by default, which is probably fine on an iPhone screen but turns out to be quite pixelated on the iPad.

Besides the resolution, you can also change the frame rate. Again, a higher value will make the game smoother, but it could also cause a performance lag. In the event that a higher frame rate encounters the bottleneck factor caused by Internet speed, the resulting gameplay will be choppy and slow, resulting in lower frame rates. So sometimes setting the Frame Rate to Standard (the default) is a better idea.
